چکیده

The determination of the state of digitalization in a given patient is often difficult and at times impossible. Although Eggleston1 indicated that the required quantity of digitalis is a function of body weight, the digitalizing dose is an empiric and occasionally a dangerous assumption. Clinically, the classical symptoms of digitalis intoxication may mimic those of circulatory failure. Nausea, vomiting, and abdominal distress may be secondary to hepatic and visceral congestion; premature ventricular systoles may be a manifestation of myocardial anoxia; and paroxysmal tachycardia may indicate excessive or inadequate digitalis administration. The electrocardiogram indicates digitalis effect, but no quantitative correlation with the S-T segment and T wave changes exists. Only with the electrocardiographic appearance of auriculoventricular block, premature ventricular systoles, bigeminus rhythm, nodal rhythm, etc., is toxicity apparent.

Effect of digitalis in patients receiving reserpine.

RECENTLY, we encountered an unusual response to digitalis. A patient with rheumatic valvular disease, congestive heart failure, and atrial fibrillation exhibited simultaneously evidence regarded by some as due to underdigitalization and by others as the result of overdigitalization. During mild exertion, the ventricular rate accelerated from 70 to 140 beats per minute with development of a puls...

sponse to additionally added digitalis, and relation of antibody effects on digitalis-induced automaticity and contracture to reversal of inotropy. Specific antibody (but not control antibody) in 1.1-1.5-fold molar excess over cardioactive steroid concentrations blocked positive inotropic effects of ouabain and acetyl strophanthidin, and gradually reversed established contractile effects of the...

The effect of acetyl strophanthidin on the hearts of normal dogs.

1. The ED50 of acetyl strophanthidin for producing ventricular arrhythmias in normal dogs was 54.5 mug/kg intravenously.2. Doses up to and including those which caused ventricular tachycardia did not produce either atrial arrhythmias or significant aberration of A-V conduction.3. Although the classic digitalis-induced rate and e.c.g. waveform changes may appear after administration of acetyl st...

Effect of Experimental Congestive Heart Failure and Acetyl Strophanthidin on Myocardial Electrolyte and Water Content.

• Changes in ionic concentration gradients across the cell membrane of various tissues after digitalis administration have been investigated intensively. Most results have indicated that the drug produces a loss of cellular potassium. On the other hand, potassium need not be lost from the myocardium following a digitalis dose which definitely increases the strength of cardiac contraction." Cell...

Plasma and myocardial digoxin concentrations in patients on oral therapy.

Methods are now available which are sensitive enough to detect digoxin concentration to levels of 5 mug./ml. Hitherto the degree of sensitivity of specific chemical methods was not acceptable for clinical application. Digoxin-specific antibodies have recently been prepared (Butler and Chen, 1967), and a test based on the radioimmunoassay principle has now been used to measure plasma digoxin lev...
